Step 1: Establish the What and Why for Change

This module will provide you with the step-by-step training & support you need to establish the focus of your change or improvement project and the reason why you are making a change. You will learn strategies that support you in your ability to:

• Explain to a colleague the reason your practice is embarking on a transformation project
• Identify team members and create a communications strategy for your transformation project
• Describe the current state of a key problem using a Process Map
• Write a SMART aim statement

Accreditation:

The American College of Physicians (ACP) is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

The ACP designates this enduring material for a maximum of 1 AMA PRA Category 1 Credit(s)^TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1 practice assessment MOC point and patient safety MOC credit in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
