Name Badges: In order to receive a customized badge, you must register by January 31, 2014. Onsite, we cannot customize your badge. If you register by this date, your badge and materials will be sent in the mail. Otherwise, they must be picked up at Physician and Guest Registration. After registering online, please send an email to Jeanine Louie-Hurd; in the Member and Customer Service Department and let her know what you want printed on your badge.
Cancellation Policy: Notice of cancellation for this meeting must be made in writing via mail, fax, or e-mail Member and Customer Service. If cancellations are received prior to March 18th, a $15 administrative fee will apply. If cancellations occur on or March 18th, no refund will be granted.
