A team of experts developed a diabetes self-management guide, Living with Diabetes: An Everyday Guide for You and Your Family. The guide covers diet, exercise, monitoring blood sugar, insulin, and pills.
Offered in English, Spanish and Chinese, the guide is written in a conversational tone at a fifth-grade reading level and includes photographs of patients with diabetes and their families. It gives practical tips and concrete examples of successful approaches.
The guide has been tested with over 200 patients, and initial feedback has been extremely positive. The Foundation hopes that the diabetes self-management guide will serve as an innovative model for patient education and empowerment.
